Documentation (of type) Contains identification and contact information for the parent of the subscribed dependent who has custody of that dependent. This component typically would be used when the custodial parent is a prior spouse of, or someone other than, the subscribed person from whom the dependent derives his or her entitlement to benefit plan coverage.
